Before you begin you start filling out the Accreditation Application, please have the following ready:

• Digital copy of your Current Business License

• Certificate of Insurance for all policies held by your company

• A list of your Insurance Provider(s), Policy Number(s), and expiration date(s)

• Digital copy of your current internal policies with respect to your practices to ensure compliance with Applicable Laws
